If you have lost your decision or it has been destroyed, you can apply for a duplicate. A duplicate is an authenticated copy of the decision that was issued in your case. The documents required to obtain a duplicate of a paper decision are: an application for the issuance of a duplicate decision, indicating the legal interest in obtaining such a duplicate - you can use the form, proof of payment of the administrative fee - you must attach proof of payment to the application for a duplicate decision, which amounts to 5 PLN for each started page of the document and is due at the time you submit your application. More information regarding the fee can be found in the fees section. The application on behalf of a minor foreigner is submitted by parents or guardians appointed by the court, or one of the parents or one of the appointed guardians. If you are using the services of a representative, you must attach a power of attorney along with the administrative fee of 17 PLN. You can also receive a duplicate of the decision if, for various reasons, you did not collect the letter with the decision, for example, if you did not inform us of your address change, the letter is considered delivered to the previous address, or if the letter was twice notified and not collected within the time set by Poczta Polska – it returns to the Office and we consider it delivered. Letters/resolutions issued by the Office in electronic form are authenticated and sent exclusively in the same form via e-Doręczenia. We encourage you to create an individual mailbox for e-Doręczenia: Create an account for e-Doręczenia – this is necessary for the delivery of letters and communication with us! Also, remember that to improve service, we have introduced a new rule: 1 electronic letter = 1 case. This will speed up the circulation of electronic documents in your case. In one letter (one electronic shipment via e-Doręczenia), send documents only for one case. Do not send documents for different cases in one shipment. The aim of the campaign is to reach a wide audience of foreigners interested in legalizing their stay in Poland with information that will increase their awareness and knowledge of the rules of entry and residence in Poland, including obtaining a residence card.
